Job overview - Regularly review risk factors and work in conjunction with other health professionals to modify care plans as appropriate In conjunction with other health professionals, assess and manage risks identified through assessment of patients / residents. Take charge of the ward/department/unit in the absence of senior nursing staff following performance review and adopt a flexible approach to hours of duty, in order to meet the needs of the service over a 24 hour period
Main duties of the job - You will be providing clinical leadership and supervision for the support workers and students whilst ensuring a comprehensive assessment is completed for the patients / residents to ensure a comprehensive person centred assessment is completed for their pathway. You will ensure each patient has an individual care plan and risk assessment, which takes into account the patient (if appropriate) and their carer’s views, with regular reviews and administer medicine within the guidelines and framework of the Nursing and Midwifery Council.
Overview of your organisation - At Eldertree we provide specialist short term high dependency support for adults with learning disabilities, autism, mental health problems and challenging behaviour including offending behaviour. Our site is split into six wards each comprising of six bedrooms.
Our aim is to help patients move into the community as soon as possible and to improve each patient’s longer term outcome. This is achieved by proving a high quality safe and secure environment that encourages the development of skills and competencies
